Minors (those under the age of 18) applying for a worker role in ministries covered by FBCK's PEACHY program must complete the PEACHY Ministry Application Form (or the PEACHY Renewal Form, if they were previously certified). In addition, a parent or legal guardian must complete the addendum below.
